What you need to know
This is a shared account. You will have access to all surveys that have been created, and all people with access will be able to see your surveys and results. If this doesn’t work for you, please let us know and we can explore alternate options.
You will login at https://surveymonkey.com using the username [email protected] and the password provided to you.
How it works
The shared account is set up using a mailing list, so that all CELS surveymonkey users will see all e-mail communications from Survey Monkey.
If you don’t have an active survey, you may want to turn off delivery of new messages. You can accomplish that at the following link:
https://lists.cels.anl.gov/mailman/listinfo/survey
The password for the above page is your personal list password you received when you were added to the list. You can have a reminder sent from that page if you don’t recall it.
You can always view the mail sent to the list at https://lists.cels.anl.gov/mailman/private/survey regardless of if you have delivery turned on or not.
Notes
Don’t share or change the surveymonkey password.
SurveyMonkey has limited the number of devices that can access a single account. As such, you may need to request an authorization e-mail from them, which will get sent to the list above. After doing so, remove the oldest computer on the list to authorize yours. This is a temporary solution and we are working on a longer-term one.

PLEASE NOTE: This service is not likely to be approved for renewal beyond the current cycle, ending March 2022. Please get familiar with Microsoft Forms (part of our Microsoft 365 subscription) for surveys, or work with CPA.
